PERMISSIONS

ONLINE PERMISSSIONS
Effective: Summer 2007

Competitions and other clubs' test sessions are requesting direct online permission approval and will not accept a "paper" form. For these competitions or tests sessions that require a direct Online Approval/permission email, please have all directed to Club Officer Dennis Gade at the email address of dupagembrshp@ameritech.net. It is imperative we keep this information centralized through Mr. Gade - please do not send requests/emails to any other Test Chair or Board Member regardless of the particular "title" or position the competition or club may ask for.

PROCEDURE FOR OBTAINING
DuPAGE FSC OUT OF CLUB PERMIT TO TEST
Effective: Summer 2007

To best accommodate our DuPage FSC members, a year round permission form for testing out of club and for competitions during the current season will be issued via email. DuPage FSC members will be able to copy the form and send it as an attachment for permissions needed for testing/and/or participation in USFS sanctioned competitions throughout the skating year.

NOTE - DuPage FSC members DO NOT NEED to attach this form for any DuPage FSC - sponsored test sessions or competitions.

To receive this "Year Round" Permission form, you must RENEW YOUR MEMBERSHIP. All DuPage memberships are due by June 1st, so please be sure that you have sent in your renewal and paid your fees. If you are unsure of your status or have not yet received your form, you may contact Mr. Dennis Gade by email at dupagembrshp@ameritech.net.

Club Policy on The Jim Mirabile Gold Skate Award Eligibility

The Jim Mirabile Gold Skate Award is a special achievement award presented to a club skating member at the club's annual awards brunch. The club skater also must have been a member for a minimum of two (2) years. Should the skater pass additional gold tests in any year, the club will have an additional plate inscribed to be added to the original plaque.

To receive "The Jim Mirabile Gold Skate Award Plaque," the skater must have passed their Senior and Junior Figures tests, Senior and Junior Freestyle tests, Senior and Junior Pair tests, or Gold and Pre-Gold Dance Tests at a DuPage FSC test session.
