to our club. The DuPage FSC does not
accept Associate memberships. If you are interested in becoming a FULL member of the DuPage
FSC, please download the appropriate membership application and follow the
instructions below/included with the file.

INSTRUCTIONS:
- Fill-out the application using the most current version of
Adobe Reader.
Turn on “Highlight Existing Fields.” Click on fields to enter
text or numbers. After completing application, save PDF file to
email and print accordingly.
- Email the completed PDF application to: DuPageFSC@hotmail.com
- Print completed PDF application and mail with check payable
to: DuPage FSC
- Make sure signature and *required information is complete prior
to mailing application and check.
- Memberships are processed in approximately 3 weeks and
confirmation will be emailed from
dupagembrshp@ameritech.net.

Upon approval, your membership will be valid from July
1st through June 30th of
the respective calendar year. PLEASE NOTE: We do not prorate any fees.
If you are renewing your membership, please contact Mr. Dennis
Gade. Please refer to your membership directory or
dupagembrshp@ameritech.net.
