In order to test on a DuPage FSC test session, the skater

submit an Out of Club permission letter that is current.
Non-members are required to pay an out of club fee of $20.00
for up to three (3) tests in the same test session.
Each test to be tried requires a separate test application
form along with a separate check. For example, if a skater wants to
take the Preliminary Moves and the Preliminary Free Skate tests, the
skater must submit two application forms and two checks – i.e. one
application and one check for the Preliminary Moves test, and a
second application and second check for the Preliminary Free Skate
test. In the event an additional test is to be attempted in the
same session, this would require a third application form and third
check to be submitted.
do we require separate forms and checks for each test?
checks are required so that in the event that a test applied for is
dropped or was contingent upon a skater passing a previous test,
that application and check will be immediately returned to the
skater on the same test date. This saves precious time and
eliminates unnecessary issuance of refund checks and mailings.
application forms also facilitate efficient scheduling of the test
session and accurate reporting of results to USFS after the test.
Test applications and checks must be mailed in one envelope
via regular U.S. Postal Service first class mail. The Test Chair
cannot sign for other delivery methods.
Applications are processed on a first come first serve
basis. Incomplete applications and incorrect fees will delay the
processing of an application. Please read the application form and
follow the instructions carefully to prevent unnecessary phone calls
and delays in processing. There is a checklist in the left lower
corner of the application that will guide you in filling out the
form accurately and completely.
No one will be allowed to test with incomplete paperwork
and/or insufficient fees. All forms and fees are to be in 20 days
before the test date. No exceptions. No forms or test fees
will be collected on the test date as this takes away from the
efficiency of the test session.
applications may be accepted; however, the skater’s application will
be placed on the “Wait List.”

event that the session is full, any wait-listed tests will be posted
on the tentative schedule.
will be notified if the skater is taken off the wait list and is
placed onto the schedule. This usually occurs when a skater who has
signed up to test the same level cancels before the test date, and
the wait-listed skater can then be placed into that open position.
note that wait-listed applicants do not automatically get scheduled

are responsible to notify the Test Chair if the skater wishes to be
placed on the next test date or to simply shred or return the
application and check back to the skater.
A tentative schedule will be posted on the DuPage FSC
website 4 to 5 days before the test date.
On the test day, please be prepared to pay for ice time and
hospitality fees. Ice time fees may be collected by our DuPage FSC
volunteer or by the ice rink staff. The ice time fee may vary and
it is not set by the DuPage FSC, it is set by the rink/facility.
With reference to a hospitality fee, this is always collected by our
DuPage FSC volunteer and ranges from $3.00 to $5.00. This fee is to
be paid in cash, as this allows for direct reimbursement to our

The fee for processing and registering the test with USFS, which is
administered by DuPage FSC. This fee is paid in advance and mailed
with the test application.
Club Fee – A surcharge charged only to those who are not DuPage FSC
members. This fee is $20.00 and good for up to three (3) tests in
the same session. This fee must be mailed with the application
form, test fee, and the Out of Club permit in the same mailing
envelope via first class mail.
Fee – Fee set by the rink and this amount may vary. In addition,
some rinks handle the collection of fees themselves, while others
may ask our DuPage FSC volunteer to collect it for them. As the
process and amounts will vary by rink facility, the ice time fees
may be paid in cash or check, and some facilities may allow for
credit card payment. This fee is incurred on the test day.
Hospitality Fee – The judges, Test Chair and assistants all
volunteer their time to judge and run the skaters’ test session. As
a courtesy and simply act of gratitude, our club provides them with
a hot breakfast, lunch or dinner. This fee is collected on the
testing day and ranges from $3.00 to $5.00. Please pay this fee in
cash, as it allows for direct reimbursement to our hospitality

to their testing time. If there are any changes in the posted
schedule from the website, the coaches will be notified at once.
Cancellations made by

occurrence) the Test Chair will contact you or your coach for other
